Capturing Patient Consent

Select the applicable tab to learn about capturing patient consent in your province.

When conducting a medication review with Patient Centre, the patient's consent must be gathered. In Ontario, patient consent must be collected and documented annually. Once collected, you will not be prompted for consent for 365 days. A prompt will appear if there is no consent on file from the last 365 days.

Patient consent can be captured electronically using a mobile tablet or signature pad. Once patient consent is collected, a copy of the signed form is saved to the Medication Review History tab.

To capture patient consent for a medication review:

  1. Launch a new medication review in Patient Centre. If consent has not been gathered in the last 365 days, a Consent window appears.

  2. Enter the following information:

    • Select Yes or No based on the patient's consent to the medication review process

    • Date

    There are two additional options that can be selected in the Consent window:

    • Prepare - temporarily bypasses the Consent window to access the medication review and prepare prior to the patient arriving. The Consent window appears the next time the medication review is launched.

    • Print - prints the MedsCheck Patient Acknowledgement of Professional Pharmacy Service form to have the patient sign a paper copy. This must be scanned and attached to the Patient Folder.

  3. Select Signature. The MedsCheck Patient Acknowledgement of Professional Pharmacy Service form appears.

  4. Select Sign. The Signature window appears.

  5. In the Patient field, have the patient sign their name.

  6. In the Comments field, enter any additional comments from the patient, if required.

  7. Select OK.

When conducting a medication review with Patient Centre, the patient's consent must be gathered.

To capture patient consent for a medication review:

  1. Launch a new medication review in Patient Centre.

  2. Select Consent.

  3. Select Yes or No based on the patient's consent to the medication review process.

  4. Select OK.
